Perspective Therapeutics is preparing for a potential Phase 3 trial of its lead-212 radiopharmaceutical candidate for neuroendocrine tumors, with site-startup preparations targeted for completion around the end of 2026 and trial initiation expected in 2027.

The company announced updated interim results from its ongoing Phase 1/2a study of [²¹²Pb]VMT-α-NET on Oct. 23. The treatment is being evaluated in patients with unresectable or metastatic neuroendocrine tumors expressing somatostatin receptor type 2 (SSTR2) and the results were presented at the European Society for Medical Oncology (ESMO) Congress in Madrid, Spain.

Perspective said regulatory discussions with the U.S. Food and Drug Administration (FDA) are underway and manufacturing capacity is scaling in parallel. The company plans to complete Phase 3 site-startup preparations around year-end, with trial initiation expected in 2027, subject to regulatory alignment.

“With regulatory engagement underway and manufacturing capacity scaling in parallel, we’re working to bring a potentially transformative new option to patients who need more choices,” said Markus Puhlmann, Chief Medical Officer of Perspective.

What This Means For Clinical Supply

Lead-212 radiopharmaceuticals bring specific timing and distribution requirements to clinical supply. Radioactive decay affects how long a product remains usable, making production schedules, transportation, and patient dosing appointments closely connected.

For a Phase 3 study, supply planning will need to account for the trial's site footprint, enrollment, dosing schedule, manufacturing capacity, and delivery requirements. The timing of site activation will also need to align with product availability.

Perspective has been expanding its regional manufacturing network alongside its clinical development program. In an August business update, the company said it expected its Chicago-area facility to complete construction in early 2027, followed by its Los Angeles-area facility in the second half of the year. The planned expansion would bring its network to four regional sites by the end of 2027.

Perspective also said it believed it had sufficient isotope access and manufacturing capacity to support ongoing studies and the planned VMT-α-NET Phase 3 trial. The company has not specified whether the new facilities will supply that study.

As Perspective moves toward Phase 3, the timing of its manufacturing expansion will be important to follow. Finishing construction is one milestone, but the facilities will still need to be ready to produce clinical supplies. The company hasn't yet shared where the Phase 3 sites will be located or how product will be distributed to them. Those details will help show how its manufacturing plans line up with the needs of the trial.

The Supply Question

How will Perspective Therapeutics coordinate isotope availability, manufacturing capacity, and site readiness as it prepares to launch its planned Phase 3 trial in 2027?
